I think the arm in the picture is just resting on the table. In order to pipe and rotate the turntable, you have to hold the pastry bag with one hand and then pipe. Otherwise, if you use two hands to pipe, you have to pipe a little, rotate the turntable a bit, pipe some more, etc.
